AHA Schedule for Quality Reviews of Victorian HACC Only Funded Organisations against the Community Care Common Standards (CCCS)
The proposed schedule for Quality Reviews of Victorian HACC Only funded organisations against the CCCS is provided in the table below. All site visits will need to be completed by April 2014 to ensure all reporting is completed by June 2014. Please note this is a proposed schedule and may need to be altered. CCCS Guide

All organisations should have previously received a copy of the Community Care Common Standards (CCCS) Guide. The CCCS Guide can be accessed and downloaded from the Department of Health and Ageing’s website

Self Assessment Tool
This is a Word Version of the Self Assessment Tool in the CCCS Guide that can be edited and used by your organisation to complete the Self Assessment Tool. If you are an organisation that receives only HACC funding (and are not ACHS/QIC accredited), you will need to use this tool to submit your completed Self Assessment to AHA.

You can download this document here

For organisations who receive both HACC and Commonwealth community care funding, you must submit your self assessment via the Department of Health and Ageing.

AHA Processes
This document describes the processes adopted by AHA to conduct the quality reviews of HACC funded organisations. These processes take into account those prescribed by the CCCS Guide as well as certain processes that are specific to the Victorian context. You can download this document here

For funded organisations that subcontract some or all of their funded activities, a process to involve subcontracted organisations in the CCCS review process has been developed. The process can be downloaded here

Victorian HACC Quality Review Resource
The Victorian HACC Quality Review Resource was developed to assist HACC funded agencies in Victoria to ensure that they are meeting specific Victorian HACC program and legislative requirements. You can download the Quality Review Resource here
